Once you do this, you will be prompted to label the field. You can use this text box to require the patient's name, medical record number, phone number, date of birth, and more. You can also enable the toggles to make the field required or configure other customizations.
Then, scroll down the customization panel and click “Save Changes” to store your settings.
Now, you can repeat this step as many times as you want to add all the required fields in the form.
Next, you may want to add a protected health information disclosure. You can add multiple “display options” to create a multiple choice field.
